Integrative DPC Membership Tiers

We now offer three distinct paths for patients seeking care. These membership options are designed to meet the diverse needs of our patients while ensuring exceptional care and access.

• Core Membership: Our foundational primary care plan. Hormone therapy and functional medicine testing and treatment is not offered at this tier.
• Signature Membership: Our most comprehensive offering. Hormone therapy (bio-identical HRT, testosterone, etc.) and functional medicine tests and treatment management are available along with primary care services.
• Hormone-Only Membership: Designed for patients seeking hormone therapy only. This plan includes a monthly management fee to cover clinical oversight, prescription management, and lab review. This plan does not include any primary care services.

Core Membership:

• Adult Core Membership (Adults 18+): $169/month
• Child* Core Membership (Age 5-17): $89/month
• Couples Core Membership (2 adults): $319/month
• Family Core Membership (2 adults and 2 children in household up to age 26): $439/month
  • Additional children: $50/month

Core Membership Includes:

• One comprehensive annual visit per year 
• Three (3) additional in-person visits for acute care or follow-ups for chronic conditions.
• Up to 8 tele-visits (video or phone visit) annually 
  • Additional in-person appointments (beyond included visits): $75/visit
  • Additional tele-consults: $50/visit
• In-office testing such as: urinalysis, strep, pap tests 
• Does not include hormone replacement therapy or advanced functional labs
• Labs/Tests offered at discounted rates.

Our primary care plan is designed for patients who mainly need regular check-ups and sometimes need care for minor illnesses or health issues. This membership is suggested for: controlled diabetes/hypertension, high cholesterol, uncomplicated pediatric patients, routine GYN care, STI checks. 

Signature Membership:

• Adult Signature Membership (Adults 18+): $249/month
• Child* Signature Membership (Age 5-17): $119/month
• Couples Signature Membership (2 adults): $449/month
• Family Signature Membership (2 Adults, 2 Children up to age 26): $599/month

• Additional children: $50/month

• 9 additional outpatient visits and unlimited telehealth appointments

• In-office testing and biopsies

• 10% discount on prescriptive/medical grade supplements

*Child membership requires an adult membership

Hormone-Only Membership:

• Adult Hormone Management Only Membership: $109/month

This plan is for patients seeking hormone therapy only. Monthly fee is a management fee to cover clinical oversight, prescription management, and lab review. Does not include any primary care/acute care visits, and any health-related concerns uncovered during hormone-therapy visits will be referred to the patient’s primary care provider. Pellet insertion, lab charges, and hormones are an additional cost.
